The anti-cardiolipin antibody (ACL) test is a blood test used to detect the presence of specific antibodies called immunoglobulin M (IgM) directed against cardiolipin, a type of phospholipid found in cell membranes. This test is commonly performed to help diagnose and monitor certain autoimmune disorders, particularly antiphospholipid syndrome (APS).

The blood sample is sent to our laboratory where it is tested for the presence of IgM antibodies against cardiolipin. The most common method used is an enzyme-linked immunosorbent assay (ELISA). In this test, the blood sample is mixed with cardiolipin molecules that are attached to a solid surface. If IgM antibodies against cardiolipin are present in the blood, they will bind to the cardiolipin molecules. This binding is then detected using specific reagents, and the reaction is measured to determine the amount of IgM antibodies.

The results of the ACL-IgM test are reported as either positive or negative. A positive result indicates the presence of IgM antibodies against cardiolipin. The presence of these antibodies, in combination with certain clinical symptoms and other diagnostic criteria, may suggest a diagnosis of antiphospholipid syndrome. However, it's important to note that a positive ACL-IgM result should be interpreted in the context of the patient's medical history, clinical presentation, and results of other tests. False positives and false negatives can occur, so additional testing and clinical evaluation are often necessary for a definitive diagnosis.
