The Anti-GBM Antibody Test, specifically measuring IgG antibodies against the glomerular basement membrane (GBM), is primarily used to evaluate certain kidney diseases, particularly Goodpasture syndrome. Conditions that may require the Anti-GBM Antibody Test include:

  • Goodpasture Syndrome: Goodpasture syndrome is a rare autoimmune disorder characterized by the presence of anti-GBM antibodies that attack the glomerular basement membrane of the kidneys, leading to glomerulonephritis (inflammation of the kidney’s filtering units) and potential lung involvement. The Anti-GBM Antibody Test is essential for confirming the diagnosis of Goodpasture syndrome.
  • Anti-GBM Disease: Anti-GBM disease, also known as anti-GBM antibody-mediated glomerulonephritis, is a condition similar to Goodpasture syndrome but without lung involvement. It is characterized by the presence of anti-GBM antibodies that target the glomerular basement membrane, leading to kidney inflammation and dysfunction. The Anti-GBM Antibody Test helps in diagnosing this condition.
  • Rapidly Progressive Glomerulonephritis (RPGN): RPGN is a type of kidney disease characterized by a rapid decline in kidney function. It can be caused by various underlying conditions, including Goodpasture syndrome, anti-GBM disease, and other immune-mediated glomerulonephritis. The Anti-GBM Antibody Test may be performed in the evaluation of RPGN to identify the presence of anti-GBM antibodies.

The Anti-GBM Antibody Test is performed by drawing a blood sample from a vein in your arm. Our phlebotomist, will clean the site with an antiseptic and apply a tourniquet to make the veins more visible. He will then insert a sterile needle into a vein and collect the required amount of blood into a collection tube.

  • Positive Result: A positive result indicates the presence of detectable levels of IgG antibodies against the GBM. This suggests an immune system response targeting the glomerular basement membrane. In the context of clinical symptoms and other diagnostic findings, a positive result is consistent with a diagnosis of Goodpasture syndrome or anti-GBM disease. Further evaluation and management are required in collaboration with a healthcare professional experienced in kidney disease.
  • Negative Result: A negative result indicates the absence or undetectable levels of IgG antibodies against the GBM. However, it’s important to note that a negative result does not definitively rule out the possibility of Goodpasture syndrome or anti-GBM disease. Some individuals with these conditions may have undetectable levels of anti-GBM antibodies in their blood at the time of testing. Additional clinical assessment, including consideration of symptoms, medical history, imaging studies, and possibly repeat testing, may be necessary to reach a conclusive diagnosis.

  • Immunosuppressive Therapy: The primary treatment for Goodpasture syndrome and anti-GBM disease involves immunosuppressive therapy. This aims to suppress the abnormal immune response and reduce the damage to the kidneys. Medications commonly used in immunosuppressive therapy may include corticosteroids, such as prednisone, along with other immunosuppressive drugs like cyclophosphamide or rituximab.
  • Plasma Exchange: Plasma exchange, also known as plasmapheresis, may be performed in severe cases of Goodpasture syndrome or anti-GBM disease. This procedure involves removing the patient’s blood plasma and replacing it with donor plasma or albumin. Plasma exchange helps remove circulating anti-GBM antibodies and other harmful substances from the blood.
